Embed a form on your website
Take a form you've already built in KudosCRM and add it to your own site — either embedded inside a page or as a clean hosted page you link to.

Once you've built a lead capture form, you need it live where your audience already is. KudosCRM gives every published form two ways to go out: an embed that renders the form inside one of your existing pages, and a hosted page on its own clean link. You pick whichever fits the spot.
This guide covers both. Either way, the form keeps its spam protection and writes every submission onto a tracked lead — with the source and landing URL attached — so you don't lose attribution by moving the form off KudosCRM.

Marketers and site owners who want KudosCRM forms live on their own site. Light familiarity with editing your website (or access to whoever does) helps.

- Access to add a snippet to your website, or somewhere to share a link.
Two ways to publish — same form
Every published form gives you both an embed and a hosted page from the same build. Use the hosted page for a link in an email, a signature, or a button; use the embed when you want visitors to fill it in without leaving your site.
Embed the form in a page
Open the form and confirm it's published
Go to Forms, open the form you want, and check that it shows Published. An unpublished form has no public link or embed yet.
Get the embed snippet
In the form's Share options, copy the embed snippet.
Paste it into your page
Add the snippet where you want the form to appear — for example on Apex Technologies' 'Request a demo' page. Most site builders (Webflow, WordPress, Squarespace, Framer) have an 'Embed' or 'Custom HTML' block; paste it there.
Publish your page
Save and publish the page on your site. The KudosCRM form now renders inline, right inside your design.
Submit a test
Fill it in as Emily Carter at Horizon Logistics and submit. Confirm a new lead appears under Leads with the source and landing URL attached.
Prefer a link? Use the hosted page
Instead of the embed, copy the hosted page link from the same Share options. Drop it into a button, an email, a chat message, or a QR code you generate with your own tool — visitors open it, submit, and land as leads with no site edit required.
What rides along automatically
- Source, campaign, and UTM captured from the page the form was submitted on — attribution isn't reconstructed by hand later.
- The form's honeypot keeps filtering bots wherever it's embedded; no captcha appears for real visitors.
- Duplicate matching, so the same person submitting twice is matched, not doubled.
- One published form reused across as many pages and links as you like — every submission still lands as a lead.
